Required before any exterior change. Fillable on screen - open it, type into the fields, save, and email it back. Rebuilt August 2026 with the current management details; regenerate with scripts/build-arc-form.py.
- Conditions for Approval (submit with your request)
Signed conditions that accompany every exterior change request. Revised May 2022.
